This is to notify you that we have received from the Internal Revenue Service a Notice of Levy in the amount of $ . We must deduct from your checks the amount necessary to satisfy the levied tax liability.
We are enclosing Form 668-W, Notice of Levy of Wages, Salary and Other Income for you to complete. The original and duplicate must be signed and returned to us within three (3) work days from the date of this letter. Your payroll checks must be held until we receive the Form 668-W from you.
This levy remains in effect for all wages and salaries until we are issued a release by the Internal Revenue Service
